Check Student Answer Sheets

When you administered the test, you should have had the students enter an identifications number. This must be at least 2 digits long, with the first digit located in the leftmost column on the grid. Check completed answer sheets for stray marks, incomplete erasures, and omitted answers.

Complete Header Sheet

Fill in all required grids (marked blue): Test Name, Test ID, Subtests, Formula Scoring, Printer, Versions, and Performance Assessment.

Test ID must be exactly 6 digits long.

Printer - On, Zone 2

Versions - No

Performance Assessment - No

Fill in optional grids beginning in leftmost column: Instructor’s Name, Test Date, School/ District, Grade/Group, and Class/Subgroup. If you specify instructor’s name, grade, and/or class, you will be able to report on those same subdivisions.

Complete Key Sheet

Complete one key sheet for each subtest. Complete the Identification # grid starting in the leftmost column with the subtest number, a blank space, the number of subtest items another blank space, and optionally, the number of correct responses required for mastery. Complete the items with the correct answers.

Batch the Forms

Arrange the forms so that the Header Sheet is first, followed by one or more Key Sheets (one for each subtest), followed by the student answer sheets. You can group the forms into a single batch or a variety of multiple batches. Place the forms in the input hopper of the scanner.
