Report a Problem

Forms to request assistance to solve problems, including requests for assistance with an office computer problem, computer labs as well as campus telephone service:

Note: If you no longer need a service that you request, please notify IT Services as soon as possible.

Faculty/staff workstation trouble
Report a problem with a faculty or staff office workstation or printer.

Computer lab or TEC trouble report
Report a problem with a computer lab workstation or a technology-enhanced classroom.

Transparency projector problem
Report a burned-out bulb or other problem with an transparency projector

  

Telephone Trouble
Report a problem with a telephone on campus, including cross-talk, damaged phones and/or cords as well as cords pulled out of jacks.

General request for assistance
Request assistance from IT Services to resolve a problem.
