Please use this form to request assistance with a faculty or staff workstation. The problem may be a hardware problem, software problem, general service request or a question about using your computer. This form starts a task in our task management software, which we regularly monitor. If you check the High Priority checkbox, a copy of the report is emailed to a permanent staff member. If your problem is an emergency or if you would like immediate assistance, please call us also at (715) 425-4357 (HELP).
