Here are some online & printable forms for requests. Some links, as indicated below, require the Adobe Acrobat Reader installed to view the form. Click here for more information.

If you have difficulty accessing or completing any of these forms, please contact the IT Services HelpDesk by sending us an online request or by calling us at (715) 425-4357 (HELP).

Note: If you no longer need a service that you request, please notify IT Services as soon as possible.

Request a Corporate Time calendar resource

Computer Installation Request (Form CIR)
Submit information necessary installation of a computer.

Computer Lab Software
Submit a request to have a computer application installed in a computer lab supported by IT Services or technology-enhanced classroom.

Computer move request (Form CM)
Do you want help moving your computer? Have you already moved? Please submit a Form CM so we know where you are and may correctly register your networked computer equipment.

Computer retention request
Submit a request to retain a computer in your department being replaced under the campus replacement program.

Class roster request
Requesting a roster for classes that you are teaching is done from within eSIS.

Email alias request
Request a generic email address which will forward to an actual uwrf email address.

Email folder sharing
Request that an email folder of yours be shared with others, or request that a generic email address which delivers to you be shared with others.

Email mailing list request
Request a discussion list for university-related topics

Network jack activation (Form JA)
Request a network jack activation in a campus building.

Network Accounts
Request various network accounts for yourself or for a guest.

Site-license software for Office or Home
Request software that is covered under a campus site-license agreement for your office or personal computer.

Technology-Enhanced Classroom Training
Request training in the proper usage of the equipment provided in the technology-enhanced classrooms.

Telephone Service Request
Request telephone services, including a new phone number, a telephone move to a new location or terminate an existing service.

Videotaping request
Request a videotaping, either in-studio or of a speaker, student presentations, or TV or satellite broadcast.

General request for service
If you have a problem, you can check for a specific trouble report. If none apply, just send us a general request for service.
